SmoothMovesYHT is a teen health promotion website. The information on this site is developed from evidence informed guidelines (what the experts say), transition research (what the research scientists discovered), and recommendations from organizations (important expert groups) such as the National Alliance to Advance Adolescent Health, Society for Adolescent Health and Medicine, the U.S. Maternal and Child Health Bureau, and the American Academy of Pediatrics. SmoothMovesYHT offers lots of expert health-smart info with a library of quick read topics.
The teen friendly web links and tools provided throughout the site were selected based on their usefulness and the expertise (know-how) of the organization or author. Many of the resources were developed in other states by expert work-groups, universities and/or government agencies. SmoothMovesYHT.org expresses great appreciation for permissions and collaborative support received from these field experts.
Mission
The mission for the SmoothMovesYHT website is pretty straightforward. It is to provide reliable resources and tools for teens seeking information on the web. SmoothMovesYHT seeks to introduce teens to the topic of transition, create role awareness, and empower teens through information.
SmoothMovesYHT 2.1 strives to become the go-to teen transition-health promotion resource! Our goal is to engage all teens with reliable information and support successful transitions to adult heath care and adulthood.
The site can also serve as an easy-to-implement patient education tool to support clinic based youth health transition programs.
The original SmoothMovesYHT.org website represents the work of a nursing doctoral student project at Louisiana State University School of Nursing - New Orleans. The site launched in December of 2015.
Mentored by Dr. Susan Berry and the Childrens Special Health Services team, this doctoral project focused on development of a health promotion resource for Louisiana's transition aged youth.
The project goal sought to orient youth to the topic of transition, create role awareness, and empower teens through knowledge.
This project would not have been possible without the shared expertise and resources provided by organizations and universities around the country and the Children's Special Health Services (CSHS) team.

The Children's Special Health Services Team worked to improve transition services and supports for the youth of Louisiana for many years and continues to do so today.

Dr. Patti Barovechio works with the Bureau of Family Health in Louisiana. She leads initiatives on developmental screening and the medical home with an emphasis on care coordination and youth health transition.

Ayris Franklin is an Audiology Doctoral student at LSU Health Sciences Center. Ayris successfully completed the 2018/19 Human Development Center Leadership Education in Neurodevelopmental and Related Disabilities (LEND) training.
Ayris hopes that her work on the SmoothmovesYHT website will help teens successfully navigate the transition period and achieve a happy and healthy adulthood.
